THIS FORM MAY NOT BE USED UNTIL BOTH TEAMS HAVE PAID - PLEASE CHECK THE CONFIRMED TEAMS LIST TO SEE IF YOUR OPPONENT HAS PAID!

Once two coaches have agreed upon a match date and time, one of the coaches should submit this form. This request will normally be booked within 48 hours of receipt. Coaches need to check the web site match schedule and report immediatley any problems. Both coaches will be sent an confirming email once the match has been added to the web site Match Schedule. Please remember that all match times and sites are subject to change. Two weeks before your arrival, there should be no more changes.
